New Patient Registration

We always welcome new patients to join us at Hackness Road Surgery. If you are in a search of a doctor in Scarborough or live in Scalby, Newby, Cloughton, Burniston or Ravenscar we can register patients who live in postcode areas YO11, YO12 and YO13.

The reception staff will provide you with a 'welcome pack'. Alternatively you can download the relevant forms from the above "Registering Online" tab. You will however still have to come to the surgery to sign the registration form and complete the new patient medical questionnaire. We kindly request that you return with your completed forms Monday-Friday between the hours of 14.00-16.00 where we will continue with the registration process.

Please bring 2 forms of Identification with you, one to include a photograph such as passport or driving licence and a utility bill to confirm your address.

We ask that you fill in all the information that we ask for so we can ensure your records are accurate and up to date.

All new patients are offered a New Patient Health Check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information and download the Patient Information Leaflet below.
